What Are Carrie’s Office Hours?
 Carrie is available Mondays through Thursdays by appointment. Evening hours are available.

What Forms of Payment Does Carrie Accept?
Carrie accepts payment by cash, check, MasterCard, Visa and Discover.

What is Carrie’s Cancellation Policy?
In order to avoid being charged, Carrie requires a 48-hour notice for cancellation of a scheduled appointment.

Does Carrie Accept Insurance?
For a variety of reasons, including those involving patient confidentiality, Carrie does not accept insurance as direct payment. Carrie requests payment at the time of service and will provide clients with statements to submit to their own insurance companies for reimbursement. Clients are responsible for determining the extent of their own insurance coverage and pre-certification requirements.
